If you married or divorced, or simply changed your name, you need to do the following:
 
 
  1. Fill out the following Reissuance of Certificate - Name Change affidavit along with $50.00 to obtain a new certificate and change your CPA record.

  2. Along with the Reissuance of Certificate - Name Change affidavit, attach a copy of the appropriate document (marriage certificate, court order, etc.)

  3. Return your old CPA certificate to the Arizona State Board of Accountancy
If using your CPA title, you must use the designation with your name as it appears on your certificate until the name change is approved by the Board.
